Narrative:

After small aircraft X departed sez airport and called on frequency, of which I didn't hear (I was busy doing other paperwork and coordination). The radar man responded and acknowledged the aircraft. Nothing else was said until phx called up wanting to know who the small aircraft was entering their airspace. That was the first I knew small aircraft X had departed. Supplemental information from acn 219513: small aircraft X departed sez on an IFR clearance and called me in the air. My attention was diverted and I did not ensure departure message was entered in computer. The aircraft entered phx approach control untracked and without a handoff.

Title: SMA X UNAUTHORIZED UNCOORDINATED PENETRATION OF PHX APCH CTL AIRSPACE. OPDEV.

Narrative: AFTER SMA X DEPARTED SEZ ARPT AND CALLED ON FREQ, OF WHICH I DIDN'T HEAR (I WAS BUSY DOING OTHER PAPERWORK AND COORD). THE RADAR MAN RESPONDED AND ACKNOWLEDGED THE ACFT. NOTHING ELSE WAS SAID UNTIL PHX CALLED UP WANTING TO KNOW WHO THE SMA WAS ENTERING THEIR AIRSPACE. THAT WAS THE FIRST I KNEW SMA X HAD DEPARTED. SUPPLEMENTAL INFO FROM ACN 219513: SMA X DEPARTED SEZ ON AN IFR CLRNC AND CALLED ME IN THE AIR. MY ATTN WAS DIVERTED AND I DID NOT ENSURE DEP MESSAGE WAS ENTERED IN COMPUTER. THE ACFT ENTERED PHX APCH CTL UNTRACKED AND WITHOUT A HDOF.
