Narrative:

There are 2 ILS approach procedures for runway 16 right at fco: ILS Z 16R and ILS Y 16R. There is only one ILS 16R choice in the FMC and it is actually the ILS Z 16R approach. The difference between the Z and Y approaches is the missed approach as far as i can see. So here is a potential scenario. The crew is cleared the ils y 16r approach and selects the ILS 16R from the box. The crew then goes missed approach and selects LNAV. The crew will then fly the wrong missed approach. A safety issue. Recommendations. 1. A NOTAM. 2. Put this information on the arrival portion of the detailed airport page. 3. Review all our airports for approaches that are similarly named but with differences and annotate the differences on the detail airport pages.

Title: An air carrier pilot discovered that his aircraft's FMC installation only contained the FCO ILS Z RWY 16R approach which has a different missed approach procedure than the ILS Y RWY 16R. Neither the FMC nor other publications stated that the only installed approach corresponded to the ILS 16R Z approach.
