Narrative:

While being vectored for the ILS to islip the visibility dropped below the minimums required, thus new york approach told us to proceed direct CCC and hold as published. I was the PF and asked my first officer to give me the radials and the direction of the hold. The first officer gave me the holding instructions that were on the approach plate as opposed to the holding pattern that was on the low altitude en route chart. The controller never questioned us and I did not realize our mistake until I reviewed the en route chart on our return flight the following day. I believe this mistake could have been prevented by greater vigilance on my part.

Title: HOLDING ON WRONG RADIALS.

Narrative: WHILE BEING VECTORED FOR THE ILS TO ISLIP THE VISIBILITY DROPPED BELOW THE MINIMUMS REQUIRED, THUS NEW YORK APCH TOLD US TO PROCEED DIRECT CCC AND HOLD AS PUBLISHED. I WAS THE PF AND ASKED MY FO TO GIVE ME THE RADIALS AND THE DIRECTION OF THE HOLD. THE FO GAVE ME THE HOLDING INSTRUCTIONS THAT WERE ON THE APCH PLATE AS OPPOSED TO THE HOLDING PATTERN THAT WAS ON THE LOW ALT ENRTE CHART. THE CTLR NEVER QUESTIONED US AND I DID NOT REALIZE OUR MISTAKE UNTIL I REVIEWED THE ENRTE CHART ON OUR RETURN FLT THE FOLLOWING DAY. I BELIEVE THIS MISTAKE COULD HAVE BEEN PREVENTED BY GREATER VIGILANCE ON MY PART.
