Narrative:

While departing felt impact. Realized that we had departed with aft crew door latched open. Door slammed shut at 400 ft AGL. We had been sitting on ground with 1 engine running. Monitoring ASOS for basic VFR conditions (terms of our maintenance ferry permit). When WX went VFR we started engines and departed. Non standard procedure had interrupted our normal checklist flow. Door was not verified closed before departure.

Title: DC6 FLT CREW TKOF WITH CABIN DOOR OPEN.

Narrative: WHILE DEPARTING FELT IMPACT. REALIZED THAT WE HAD DEPARTED WITH AFT CREW DOOR LATCHED OPEN. DOOR SLAMMED SHUT AT 400 FT AGL. WE HAD BEEN SITTING ON GND WITH 1 ENG RUNNING.
