Failed to maintain 1500 ft until the 4.5 teb DME as prescribed in the teb 5 departure. No excuses we have flown this departure many times and just plain forgot.

Title: A C560XL DEPARTING TEB; LEVELED AT 2000 FT BEFORE PASSING 4.5 DME.
Narrative: FAILED TO MAINTAIN 1500 FT UNTIL THE 4.5 TEB DME AS PRESCRIBED IN THE TEB 5 DEP. NO EXCUSES WE HAVE FLOWN THIS DEP MANY TIMES AND JUST PLAIN FORGOT.
