Narrative:

During preflight for flight xyz, we had multiple interruptions and distrs while completing our checklist. While I checked the maintenance log for maintenance status, I missed the last page which was behind the cardboard cover. I initialed the wrong page and we departed ZZZ with 3 minor maintenance items open that should have had a balancing entry. The good news is that all 3 items written up worked fine on our leg. This could have been a far more serious deal and I will not let distrs keep me from finding the last page of the log in the future. All items were again debriefed and balanced by maintenance at XXX.

Title: A B757-200 WAS DISPATCHED IN NON COMPLIANCE WITH OPEN MAINT LOG RPTS NEITHER DEFERRED NOR ANSWERED.

Narrative: DURING PREFLT FOR FLT XYZ, WE HAD MULTIPLE INTERRUPTIONS AND DISTRS WHILE COMPLETING OUR CHKLIST. WHILE I CHKED THE MAINT LOG FOR MAINT STATUS, I MISSED THE LAST PAGE WHICH WAS BEHIND THE CARDBOARD COVER. I INITIALED THE WRONG PAGE AND WE DEPARTED ZZZ WITH 3 MINOR MAINT ITEMS OPEN THAT SHOULD HAVE HAD A BALANCING ENTRY. THE GOOD NEWS IS THAT ALL 3 ITEMS WRITTEN UP WORKED FINE ON OUR LEG. THIS COULD HAVE BEEN A FAR MORE SERIOUS DEAL AND I WILL NOT LET DISTRS KEEP ME FROM FINDING THE LAST PAGE OF THE LOG IN THE FUTURE. ALL ITEMS WERE AGAIN DEBRIEFED AND BALANCED BY MAINT AT XXX.
