Narrative:

I discovered on mar/xa/00 that my temporary airmans certificate had expired. I had flown 1 trip with expired certificate. I went to my air carrier and was told to go to FAA and receives an emergency temporary certificate, which I did. On checking why I hadn't received my permanent license, we discovered that my license had been mailed to my street address but was returned to FAA because my mail goes to a po box. I should have been aware that my license was to expire, but I was thinking it was for 6 months and it was for 4 months. Also, the form requires a street address and does not allow for po box addresses. I would recommend 2 addresses to be allowed -- one for physical address and the other for po box addresses.

Title: B777 CAPT FLEW TRIP WITH EXPIRED TEMPORARY AIRMANS CERTIFICATE.

Narrative: I DISCOVERED ON MAR/XA/00 THAT MY TEMPORARY AIRMANS CERTIFICATE HAD EXPIRED. I HAD FLOWN 1 TRIP WITH EXPIRED CERTIFICATE. I WENT TO MY ACR AND WAS TOLD TO GO TO FAA AND RECEIVES AN EMERGENCY TEMPORARY CERTIFICATE, WHICH I DID. ON CHKING WHY I HADN'T RECEIVED MY PERMANENT LICENSE, WE DISCOVERED THAT MY LICENSE HAD BEEN MAILED TO MY STREET ADDRESS BUT WAS RETURNED TO FAA BECAUSE MY MAIL GOES TO A PO BOX. I SHOULD HAVE BEEN AWARE THAT MY LICENSE WAS TO EXPIRE, BUT I WAS THINKING IT WAS FOR 6 MONTHS AND IT WAS FOR 4 MONTHS. ALSO, THE FORM REQUIRES A STREET ADDRESS AND DOES NOT ALLOW FOR PO BOX ADDRESSES. I WOULD RECOMMEND 2 ADDRESSES TO BE ALLOWED -- ONE FOR PHYSICAL ADDRESS AND THE OTHER FOR PO BOX ADDRESSES.
