Narrative:

On the morning of aug/xa/98, flight xyz from ZZZ to sju air- interrupted due to low oil pressure, causing the crew to shut down the left engine and return to the station. Upon further examination of the engine, it was found that the oil cap was off. The preceding night I had the task of conducting the inspection and servicing of that engine. I svced the engine with 1 pint of oil. To the best of my knowledge, I replaced the cap on the engine.

Title: A B757-200 RETURNED TO THE FIELD DUE TO #1 ENG LOSS OF OIL PRESSURE AND QUANTITY CAUSED BY THE OIL FILLER CAP NOT SECURED.

Narrative: ON THE MORNING OF AUG/XA/98, FLT XYZ FROM ZZZ TO SJU AIR- INTERRUPTED DUE TO LOW OIL PRESSURE, CAUSING THE CREW TO SHUT DOWN THE L ENG AND RETURN TO THE STATION. UPON FURTHER EXAMINATION OF THE ENG, IT WAS FOUND THAT THE OIL CAP WAS OFF. THE PRECEDING NIGHT I HAD THE TASK OF CONDUCTING THE INSPECTION AND SVCING OF THAT ENG. I SVCED THE ENG WITH 1 PINT OF OIL. TO THE BEST OF MY KNOWLEDGE, I REPLACED THE CAP ON THE ENG.
