Narrative:

Flight departed rdu on the pack 4 departure. We were given several turns leaving rdu and leveloffs, a little distracting. Our last turn was to intercept the pack 4 west of rdu about 20 mi out. We then crossed over liberty VOR and I started a turn to intercept the 169 degree radial of the pulaski VOR (psk) per our flight plan but instead looking at the pack 4 was deceived into thinking the roa 170 degree radial was the psk radial. The format of the pack 4 is very misleading and company does in fact have a NOTAM on the departure specifically for the radial to azell intersection. I believe the correction on the departure could be to put psk VOR on the SID instead of roa or to include both of them. Thank you for your time and consideration in this matter.

Title: MLG FLC TURNS TO WRONG HDG WHEN CLRED AS FILED ON DEP SID. SELECTED WRONG VOR.

Narrative: FLT DEPARTED RDU ON THE PACK 4 DEP. WE WERE GIVEN SEVERAL TURNS LEAVING RDU AND LEVELOFFS, A LITTLE DISTRACTING. OUR LAST TURN WAS TO INTERCEPT THE PACK 4 W OF RDU ABOUT 20 MI OUT. WE THEN CROSSED OVER LIBERTY VOR AND I STARTED A TURN TO INTERCEPT THE 169 DEG RADIAL OF THE PULASKI VOR (PSK) PER OUR FLT PLAN BUT INSTEAD LOOKING AT THE PACK 4 WAS DECEIVED INTO THINKING THE ROA 170 DEG RADIAL WAS THE PSK RADIAL. THE FORMAT OF THE PACK 4 IS VERY MISLEADING AND COMPANY DOES IN FACT HAVE A NOTAM ON THE DEP SPECIFICALLY FOR THE RADIAL TO AZELL INTXN. I BELIEVE THE CORRECTION ON THE DEP COULD BE TO PUT PSK VOR ON THE SID INSTEAD OF ROA OR TO INCLUDE BOTH OF THEM. THANK YOU FOR YOUR TIME AND CONSIDERATION IN THIS MATTER.
