Narrative:

Air carrier X progressed ground intersection and entered ZNY airspace on the wrong arwy. The aircraft departed V. C. Bird international and was coordinated with san juan center over gabar intersection. This was also an error. The aircraft showed up over totem intersection, causing another system error with mlt Y crossing west to east at FL220. The error at totem was reported to the area supervisor, and being handled according to procedure. The deviation at grann intersection with ZNY was not reported to management. I received the call from ZNY when he received the pilot reports from arinc. Frequently there may be multiple flight plans on an aircraft requiring a full route clearance be issued. Not done in this event. Even after the first error occurred at totem, no action was taken to ensure the complete correct routing was mutually understood. The sector was busy at that time and the controllers failed to issue the full route clearance. Callback conversation with reporter revealed the following: reporter stated air carrier X was a 2 engine widebody transport. Sep standard between air carrier and mlt Y should have been 15 mins. 10 mins sep existed. The clearance delivery controller at V. C. Bird tower issued 1 clearance but coordination another with san juan. The radar controller at san juan failed to issue a full route clearance when air carrier X showed up in the wrong place. The reporter stated he should have verified the routing with V. C. Bird tower.

Title: ACR X HAD LESS THAN STANDARD SEPARATION FROM MLT Y. SYSTEM ERROR.

Narrative: ACR X PROGRESSED GND INTXN AND ENTERED ZNY AIRSPACE ON THE WRONG ARWY. THE ACFT DEPARTED V. C. BIRD INTL AND WAS COORDINATED WITH SAN JUAN CENTER OVER GABAR INTXN. THIS WAS ALSO AN ERROR. THE ACFT SHOWED UP OVER TOTEM INTXN, CAUSING ANOTHER SYS ERROR WITH MLT Y XING W TO E AT FL220. THE ERROR AT TOTEM WAS RPTED TO THE AREA SUPVR, AND BEING HANDLED ACCORDING TO PROC. THE DEVIATION AT GRANN INTXN WITH ZNY WAS NOT RPTED TO MGMNT. I RECEIVED THE CALL FROM ZNY WHEN HE RECEIVED THE PLT RPTS FROM ARINC. FREQUENTLY THERE MAY BE MULTIPLE FLT PLANS ON AN ACFT REQUIRING A FULL RTE CLRNC BE ISSUED. NOT DONE IN THIS EVENT. EVEN AFTER THE FIRST ERROR OCCURRED AT TOTEM, NO ACTION WAS TAKEN TO ENSURE THE COMPLETE CORRECT RTING WAS MUTUALLY UNDERSTOOD.
