Narrative:

Small transport X while under the control of denver approach control made a missed approach midway down runway 29R at the jeffco airport. The missed approach was made because there was an aircraft on the runway. The small aircraft Y aircraft on the runway was on departure roll, small transport X was suppose to be making an ILS approach to runway 29R at jeffco but was not given a frequency change to the jeffco control tower until he advised denver approach control that he was making a missed approach because of small aircraft Y on the runway. The jeffco tower controller became aware of small transport X when he was on short final. The tower controller gave small transport X radio instructions to go around, aircraft on the runway. Small transport X did not respond or answer any radio calls. The tower controller then gave small transport X a red light gun signal for go around. I believe denver approach control may not have given small transport X a frequency change before he started an ILS final approach, or the aircraft turned onto final while being vectored to the ILS final by approach control. He did not have clearance to land on runway 29R. The jeffco tower controller required immediate relief from the control position because of the closeness of the aircraft.

Title: SMT X FAILED TO CONTACT TWR AT FAF HAD NMAC WITH SMA Y ON TKOF. PLTDEV.

Narrative: SMT X WHILE UNDER THE CTL OF DENVER APCH CTL MADE A MISSED APCH MIDWAY DOWN RWY 29R AT THE JEFFCO ARPT. THE MISSED APCH WAS MADE BECAUSE THERE WAS AN ACFT ON THE RWY. THE SMA Y ACFT ON THE RWY WAS ON DEP ROLL, SMT X WAS SUPPOSE TO BE MAKING AN ILS APCH TO RWY 29R AT JEFFCO BUT WAS NOT GIVEN A FREQ CHANGE TO THE JEFFCO CTL TWR UNTIL HE ADVISED DENVER APCH CTL THAT HE WAS MAKING A MISSED APCH BECAUSE OF SMA Y ON THE RWY. THE JEFFCO TWR CTLR BECAME AWARE OF SMT X WHEN HE WAS ON SHORT FINAL. THE TWR CTLR GAVE SMT X RADIO INSTRUCTIONS TO GO AROUND, ACFT ON THE RWY. SMT X DID NOT RESPOND OR ANSWER ANY RADIO CALLS. THE TWR CTLR THEN GAVE SMT X A RED LIGHT GUN SIGNAL FOR GO AROUND. I BELIEVE DENVER APCH CTL MAY NOT HAVE GIVEN SMT X A FREQ CHANGE BEFORE HE STARTED AN ILS FINAL APCH, OR THE ACFT TURNED ONTO FINAL WHILE BEING VECTORED TO THE ILS FINAL BY APCH CTL. HE DID NOT HAVE CLRNC TO LAND ON RWY 29R. THE JEFFCO TWR CTLR REQUIRED IMMEDIATE RELIEF FROM THE CTL POS BECAUSE OF THE CLOSENESS OF THE ACFT.
