Narrative:

Flight cvg-pit arrived pit xa:00 local after a 5 hour ground hold in cvg due to fog at pit. Landed on 28L. Taxi back via C to follow an medium large transport holding short of J. As medium large transport passed J on C first officer switched to talk to company. Captain took control of radios for ground operation (we split the radios). While first officer was talking to company he observed captain talking to the tower and his continued taxi. The aircraft continued down C with a left turn on the new 32 and crossed 28C. The first officer returned to tower and radio and was asked why flight crossed 28C an active runway. No response was given, then a switch to ground. Flight continue under instruction to the gate. With so many aircraft on the ground west/O gates. Callsigns and aircraft types were questioned often. Consideration/performance. There is alot of construction and non standard taxi routes at pit while 32 and txwys are being rebuilt. This occurred after a difficult WX day at pit (zero/zero) for controllers and pilots alike. The instructions to follow the medium large transport and hold short or follow the medium large transport holding short may have been misunderstood by both parties. The crew did split the radio responsibilities when the first officer went to company radio (a good procedure) however? The first officer although talking to company, did not question the aircraft movement because he saw the captain talking to the tower and he was following the daisy chain to the gate. Callback conversation with reporter revealed the following: reporter participated in the boeing study. Captain, PF, had such an earache he was displaced from the flight and took ground transportation to his home domicile.

Narrative: FLT CVG-PIT ARRIVED PIT XA:00 LCL AFTER A 5 HOUR GND HOLD IN CVG DUE TO FOG AT PIT. LANDED ON 28L. TAXI BACK VIA C TO FOLLOW AN MLG HOLDING SHORT OF J. AS MLG PASSED J ON C F/O SWITCHED TO TALK TO COMPANY. CAPT TOOK CTL OF RADIOS FOR GND OPERATION (WE SPLIT THE RADIOS). WHILE F/O WAS TALKING TO COMPANY HE OBSERVED CAPT TALKING TO THE TWR AND HIS CONTINUED TAXI. THE ACFT CONTINUED DOWN C WITH A L TURN ON THE NEW 32 AND CROSSED 28C. THE F/O RETURNED TO TWR AND RADIO AND WAS ASKED WHY FLT CROSSED 28C AN ACTIVE RWY. NO RESPONSE WAS GIVEN, THEN A SWITCH TO GND. FLT CONTINUE UNDER INSTRUCTION TO THE GATE. WITH SO MANY ACFT ON THE GND W/O GATES. CALLSIGNS AND ACFT TYPES WERE QUESTIONED OFTEN. CONSIDERATION/PERFORMANCE. THE F/O ALTHOUGH TALKING TO COMPANY, DID NOT QUESTION THE ACFT MOVEMENT BECAUSE HE SAW THE CAPT TALKING TO THE TWR AND HE WAS FOLLOWING THE DAISY CHAIN TO THE GATE. CALLBACK CONVERSATION WITH REPORTER REVEALED THE FOLLOWING: RPTR PARTICIPATED IN THE BOEING STUDY. CAPT, PF, HAD SUCH AN EARACHE HE WAS DISPLACED FROM THE FLT AND TOOK GND TRANSPORTATION TO HIS HOME DOMICILE.
