Narrative:

During takeoff on runway 13 lga, aircraft hit an unknown object causing an extreme vertical bump which sprained my cervical. Subsequent investigation revealed that a wheel chock was discovered by the lga port authority in the vicinity of our occurrence. Runways should be swept more often. Callback conversation with reporter revealed the following: reporter went to the hospital after arrival in mia. Returned to ny on the subsequent ferry flight. Is still receiving medical attention for his back. Discussed with tower and they advised nothing was observed. On a subsequent runway sweep a wheel chock was found to the side of the runway in the approximately position where they hit the object. Port authority has made no changes in procedures. Company found no damage to the aircraft on check at mia.

DISCUSSED WITH TWR AND THEY ADVISED NOTHING WAS OBSERVED. ON A SUBSEQUENT RWY SWEEP A WHEEL CHOCK WAS FOUND TO THE SIDE OF THE RWY IN THE APPROX POS WHERE THEY HIT THE OBJECT. PORT AUTHORITY HAS MADE NO CHANGES IN PROCS. COMPANY FOUND NO DAMAGE TO THE ACFT ON CHK AT MIA.
