Narrative:

Small transport X was asked to report 2 mi final for runway 03. He failed to do so and made low approach without clearance which caused a confliction with small aircraft Y departing runway 34. Evasive action was taken by ATC to avoid the conflict at the mid-field intersection. Callback conversation with reporter revealed the following: reporter spotted the small transport on low approach at about a half mi final. He gave the small aircraft on takeoff roll as traffic, and instructed the small transport to make an early turn to the right and start his climb. The small transport turned as instructed and passed behind the small aircraft which was still on the runway surface on takeoff roll. No loss of separation occurred and the small transport pilot was advised that he failed to report on 2 mi final as instructed. Nothing more was said or done concerning this incident. Reporter's experience level was ascertained.

Title: GOV SMT MADE UNAUTH LOW APCH AND NEARLY CAUSED CONFLICT WITH SMA ON TKOF ROLL.

Narrative: SMT X WAS ASKED TO REPORT 2 MI FINAL FOR RWY 03. HE FAILED TO DO SO AND MADE LOW APCH WITHOUT CLRNC WHICH CAUSED A CONFLICTION WITH SMA Y DEPARTING RWY 34. EVASIVE ACTION WAS TAKEN BY ATC TO AVOID THE CONFLICT AT THE MID-FIELD INTXN. CALLBACK CONVERSATION WITH REPORTER REVEALED THE FOLLOWING: REPORTER SPOTTED THE SMT ON LOW APCH AT ABOUT A HALF MI FINAL. HE GAVE THE SMA ON TKOF ROLL AS TFC, AND INSTRUCTED THE SMT TO MAKE AN EARLY TURN TO THE RIGHT AND START HIS CLIMB. THE SMT TURNED AS INSTRUCTED AND PASSED BEHIND THE SMA WHICH WAS STILL ON THE RWY SURFACE ON TKOF ROLL. NO LOSS OF SEPARATION OCCURRED AND THE SMT PLT WAS ADVISED THAT HE FAILED TO REPORT ON 2 MI FINAL AS INSTRUCTED. NOTHING MORE WAS SAID OR DONE CONCERNING THIS INCIDENT. REPORTER'S EXPERIENCE LEVEL WAS ASCERTAINED.
