Narrative:

Instructed to taxi to xxl for departure. I instructed the first officer to 'sit the flight attendants down' and to do the 'before takeoff checklist' he advised me the purser was still on the PA. The before takeoff checklist was completed and the first officer made the 'flight attendants prepare for take off' announcement. An rj was number one for take off; the tower called the rj three times before he answered. The rj accepted the takeoff clearance. We were instructed to 'taxi into position and hold xxl'. As we lined up we were given a takeoff clearance. We conducted a normal takeoff. After we were airborne for a while the first flight attendant called. She informed us that they did not receive the 'prepare for takeoff' command. The first officer had used the hand held mike. We tested the hand held mike with ATC and found it to be inoperative. I asked the first flight attendant if anyone was hurt and was informed some flight attendants were not in position and one was in a lavatory. No injuries were reported. We wrote up the mike and advised maintenance.

Title: A B767 took off with the Flight Attendants unseated when the prepare for takeoff announcement was delivered via a hand held microphone later determined to be inoperative.
