Narrative:

Scheduled plane change in phx. Arrived at new aircraft with plenty of time to prepare. While preflting aircraft the MCP failed and required maintenance replacement. Although we still had time to perform our duties; it appears the distraction resulted in the first officer's failure to notice that our pre departure clearance had changed from the maxxo as filed to the chezz. He copied it down incorrectly onto the pre departure clearance sheet attached to the WX pack and loaded the information into the navigation computer. Our before start checklist and predep brief were in accordance with the fom with review of the route and its proper entry. On departure; ATC vectored us to a point on the chezz departure and this is where we noticed our error. When we called up the pre departure clearance; it did show the chezz departure versus the maxxo. The rest of the leg uneventful. The presentation of revised clrncs in pre departure clearance's has obviously been a source of much confusion. Have we considered modifying the pre departure clearance page to eliminate the confusion? I would suggest 1 of 2 things: 1) only list the assigned clearance on the pre departure clearance. They should note the discrepancy between the printed release and the computer generated pre departure clearance. 2) list the revised clearance on page 1 of the computer generated pre departure clearance release and the original on page 2; thus further highlighting the change.

Title: ACR DEP FROM PHX FAILED TO NOTE PDC CHANGE FROM MAXXO TO CHEZZ RESULTING IN TRACK ERROR; REPORTER SUGGESTED PDC FORMAT CHANGE.

OUR BEFORE START CHKLIST AND PREDEP BRIEF WERE IN ACCORDANCE WITH THE FOM WITH REVIEW OF THE RTE AND ITS PROPER ENTRY.
