Narrative:

During a scheduled gear change the landing gear struts were svced. The task card used to change the gear refers the technician to the maintenance manual to service. It requires a check of the struts within 10 flts to verify condition of strut service. The log page inadvertently closed without filling out the required deferral to check the struts as required. I failed to recognize the missing log page when reviewing the work package prior to signing for the airworthy release.

Title: A B737-400 WAS DISPATCHED AFTER A SCHEDULED LNDG GEAR CHANGE AND SVC TECHNICIAN FAILED TO ENTER DEFERRED ITEM TO CHK SVC WITHIN 10 FLTS.

Narrative: DURING A SCHEDULED GEAR CHANGE THE LNDG GEAR STRUTS WERE SVCED. THE TASK CARD USED TO CHANGE THE GEAR REFERS THE TECHNICIAN TO THE MAINT MANUAL TO SVC. IT REQUIRES A CHK OF THE STRUTS WITHIN 10 FLTS TO VERIFY CONDITION OF STRUT SVC. THE LOG PAGE INADVERTENTLY CLOSED WITHOUT FILLING OUT THE REQUIRED DEFERRAL TO CHK THE STRUTS AS REQUIRED.
