Narrative:

ATIS reported runway 21 in use; ground cleared us to taxi to runway 3 hold short at taxiway D. While holding; ground control xferred us to tower frequency. Tower cleared us to: 'north-number; back-taxi approved; when ready fly runway heading; cleared for takeoff.' runway was not specified. Since ground cleared us to runway 3; we assumed the back-taxi was for runway 3. As we took off we noticed another plane turning short final for runway 21 directly at us; we took evasive action to the right. Tower then instructed cherokee on short final to go around and take an immediate right turn. Tower apologized for the miscom. Other aircraft was no closer than 1 mi and the TCAS never gave a traffic alert.

TWR CLRED US TO: 'N-NUMBER; BACK-TAXI APPROVED; WHEN READY FLY RWY HDG; CLRED FOR TKOF.' RWY WAS NOT SPECIFIED. SINCE GND CLRED US TO RWY 3; WE ASSUMED THE BACK-TAXI WAS FOR RWY 3. AS WE TOOK OFF WE NOTICED ANOTHER PLANE TURNING SHORT FINAL FOR RWY 21 DIRECTLY AT US; WE TOOK EVASIVE ACTION TO THE R. TWR THEN INSTRUCTED CHEROKEE ON SHORT FINAL TO GO AROUND AND TAKE AN IMMEDIATE R TURN. TWR APOLOGIZED FOR THE MISCOM. OTHER ACFT WAS NO CLOSER THAN 1 MI AND THE TCAS NEVER GAVE A TFC ALERT.
