Narrative:

During preflight inspection, I noted a 1.5 inch deformity on a #1 engine fan blade. The deformity was on a leading edge at mid-span and appeared to be the result of a small bird strike. The word 'ok' was written next to the deformity on the fan blade. I notified the captain and we checked the logbook. In the deferred maintenance section, we found the following entry: #1 engine requires borescope inspection within next 10 cycles for bird strike.' below was written: 'borescope performed. No defects noted.' clearance XXXX apr/xa/04. We discussed the write-up and concluded the engine and aircraft was good to fly. Later, after further discussion on the 3RD leg of the day, we reexamined the logbook and noted that there was no specific reference to the fan blade -- only that the engine had 'no defects.' there was nothing indicating the fan blade had been damaged and then later cleared for flight. We agreed that an 'ok' on the fan blade without documentation in the logbook is probably insufficient. The information should have been annotated in the logbook for flight crew review.

Title: A B737-300 WAS FOUND TO HAVE 1 FAN BLADE ON THE L ENG WITH THE LEADING EDGE CURLED AT MID SPAN. NO DOCUMENTATION ON BLADE DAMAGE FOUND IN LOGBOOK.

CLRNC XXXX APR/XA/04. WE DISCUSSED THE WRITE-UP AND CONCLUDED THE ENG AND ACFT WAS GOOD TO FLY. LATER, AFTER FURTHER DISCUSSION ON THE 3RD LEG OF THE DAY, WE REEXAMINED THE LOGBOOK AND NOTED THAT THERE WAS NO SPECIFIC REF TO THE FAN BLADE -- ONLY THAT THE ENG HAD 'NO DEFECTS.' THERE WAS NOTHING INDICATING THE FAN BLADE HAD BEEN DAMAGED AND THEN LATER CLRED FOR FLT. WE AGREED THAT AN 'OK' ON THE FAN BLADE WITHOUT DOCUMENTATION IN THE LOGBOOK IS PROBABLY INSUFFICIENT. THE INFO SHOULD HAVE BEEN ANNOTATED IN THE LOGBOOK FOR FLT CREW REVIEW.
