Narrative:

This report is a timeline of events leading to my 'fatigue call' while on reserve on aug/sun/03. Previous week, captain trip in mco total block time 20 hours 24 mins when I arrive home, crew scheduling calls to assign me a 2-DAY trip starting aug/fri/03, report time was XB00. On aug/fri/03 through aug/sun/03, I have scheduled reserve, on a pm. The trip I was assigned on aug/fri/03, flew 4 legs. With delays we terminated at fll at XM10, and our daily block time was 6 hours 47 mins. The next day, aug/sat/03, I was scheduled to fly 1 leg from fll to bwi in the late afternoon, departing the hotel approximately XG30. At about XA15 that morning I was awake but had not showered or anything yet. I was called by crew scheduling to see if I can be ready in 30 mins to catch an airport van to deadhead to a waiting flight to isp, then deadhead to bwi which was delayed waiting for me also. Next I would fly from bwi to bhm. I explained that I had not showered or anything yet and I would do my best. I made the flts that were asked of me, without eating breakfast, or lunch. During our mechanical delay in bwi, I did manage to quickly choke down a soggy tuna sandwich. We finally managed to arrive in bhm at XK53, securing the terminating aircraft. My block time for this day was 1 hour 53 mins. I waited for the hotel, which was not expecting me, and 'knew nothing about a pilot from our company or flight number.' upon arrival at the hotel, I find it bustling with activity and loud crowds. It seems that there is a concert there or next door and the hotel is packed with concert goers. I waited in line to check in, and politely asked for a quiet room since I have been assigned a deadhead back to bwi, van time at XB30 on aug/sun/03, the lady at the front desk said, 'just call us if it's noisy and we'll send security.' I arrive at my room and find it is occupied. I went back downstairs and got another room. I finally got to my actual room at XL45. I had not eaten dinner, and I was too tired to get food now. The night was filled with thumping of bass speaker sounds, and loud people. I was awakened several times on this short night, and at XA30 (my alarm time), I felt really worn out. I checked in for my deadhead flight to bwi, and then called scheduling. I spoke to a female crew scheduler and asked if I would be released upon arriving at bwi because I had already flown 29 plus hours in the last 5 days. I may have told her about the noise at the hotel and not getting any sleep, but can't recall for sure. I was told that I was still good for 54 mins and I am on reserve today. I stated that I was too tired and would not fly today.

NEXT I WOULD FLY FROM BWI TO BHM. I EXPLAINED THAT I HAD NOT SHOWERED OR ANYTHING YET AND I WOULD DO MY BEST. I MADE THE FLTS THAT WERE ASKED OF ME, WITHOUT EATING BREAKFAST, OR LUNCH. DURING OUR MECHANICAL DELAY IN BWI, I DID MANAGE TO QUICKLY CHOKE DOWN A SOGGY TUNA SANDWICH. WE FINALLY MANAGED TO ARRIVE IN BHM AT XK53, SECURING THE TERMINATING ACFT. UPON ARR AT THE HOTEL, I FIND IT BUSTLING WITH ACTIVITY AND LOUD CROWDS. IT SEEMS THAT THERE IS A CONCERT THERE OR NEXT DOOR AND THE HOTEL IS PACKED WITH CONCERT GOERS. I WAITED IN LINE TO CHK IN, AND POLITELY ASKED FOR A QUIET ROOM SINCE I HAVE BEEN ASSIGNED A DEADHEAD BACK TO BWI, VAN TIME AT XB30 ON AUG/SUN/03, THE LADY AT THE FRONT DESK SAID, 'JUST CALL US IF IT'S NOISY AND WE'LL SEND SECURITY.' I ARRIVE AT MY ROOM AND FIND IT IS OCCUPIED. I WENT BACK DOWNSTAIRS AND GOT ANOTHER ROOM. I FINALLY GOT TO MY ACTUAL ROOM AT XL45. I HAD NOT EATEN DINNER, AND I WAS TOO TIRED TO GET FOOD NOW.
