Narrative:

After landing on runway 25L, tower cleared us to exit at F4 and cross runway 25R (we thought). I took control of the aircraft and first officer read back 'cross runway 25R at F4, stay with you.' on taxiway east, we asked tower if we should switch to ground. Tower told us he wanted us to hold short of runway 25R, but go ahead and switch to ground. Contributing factors: we were in full reverse when the clearance was given. I took control of the aircraft to exit and first officer made the readback. According to the tower manager, the controller cleared us to hold short of runway 25R on F4, but the first officer read back 'runway 25R at F4 stay with you.' I believe that it must have been the noise level and activity of transition of aircraft control that caused our misunderstanding of the clearance and that in transition, the first officer may have started his readback before keying the microphone. We both thought the clearance was cross and we both noticed and commented on an aircraft in position with lights on prior to crossing. I thought the first officer had read back the full clearance as we understood. Corrective action: I will request confirmation of future clrncs received when noise and activity levels are high. I will also consider purchasing a noise canceling headset. Supplemental information from acn 589028: I was on landing rollout in reverse thrust when tower gave the instructions to clear runway 25L. I thought the taxi instructions stated, 'exit at F4, cleared across runway 25R, remain on this frequency.' (it was discovered later that the actual instructions were to exit at F4, hold short of runway 25R, remain on this frequency.) I read back the taxi instructions as I thought I heard them. No correction was made by tower. We both noticed an aircraft in the takeoff position but not moving. As we started to cross runway 25R, I noted that the aircraft had turned on their takeoff/landing lights. The departing aircraft did not have to abort takeoff since power had not been advanced. Tape revealed I never said 'hold short.' personal stress level was high -- lots of fatigue.

Title: B737-300 CREW HAD A RWY INCURSION AT PHX. TAXI CLRNC WAS ISSUED DURING REVERSE THRUST ROLLOUT AND HOLD SHORT CLRNC WAS MISSED BY FO DURING TRANSITION OF RESPONSIBILITIES TO CAPT.

Narrative: AFTER LNDG ON RWY 25L, TWR CLRED US TO EXIT AT F4 AND CROSS RWY 25R (WE THOUGHT). I TOOK CTL OF THE ACFT AND FO READ BACK 'CROSS RWY 25R AT F4, STAY WITH YOU.' ON TXWY E, WE ASKED TWR IF WE SHOULD SWITCH TO GND. TWR TOLD US HE WANTED US TO HOLD SHORT OF RWY 25R, BUT GO AHEAD AND SWITCH TO GND. CONTRIBUTING FACTORS: WE WERE IN FULL REVERSE WHEN THE CLRNC WAS GIVEN. I TOOK CTL OF THE ACFT TO EXIT AND FO MADE THE READBACK. ACCORDING TO THE TWR MGR, THE CTLR CLRED US TO HOLD SHORT OF RWY 25R ON F4, BUT THE FO READ BACK 'RWY 25R AT F4 STAY WITH YOU.' I BELIEVE THAT IT MUST HAVE BEEN THE NOISE LEVEL AND ACTIVITY OF TRANSITION OF ACFT CTL THAT CAUSED OUR MISUNDERSTANDING OF THE CLRNC AND THAT IN TRANSITION, THE FO MAY HAVE STARTED HIS READBACK BEFORE KEYING THE MIKE. WE BOTH THOUGHT THE CLRNC WAS CROSS AND WE BOTH NOTICED AND COMMENTED ON AN ACFT IN POS WITH LIGHTS ON PRIOR TO XING. I THOUGHT THE FO HAD READ BACK THE FULL CLRNC AS WE UNDERSTOOD. CORRECTIVE ACTION: I WILL REQUEST CONFIRMATION OF FUTURE CLRNCS RECEIVED WHEN NOISE AND ACTIVITY LEVELS ARE HIGH. I WILL ALSO CONSIDER PURCHASING A NOISE CANCELING HEADSET. SUPPLEMENTAL INFO FROM ACN 589028: I WAS ON LNDG ROLLOUT IN REVERSE THRUST WHEN TWR GAVE THE INSTRUCTIONS TO CLR RWY 25L. I THOUGHT THE TAXI INSTRUCTIONS STATED, 'EXIT AT F4, CLRED ACROSS RWY 25R, REMAIN ON THIS FREQ.' (IT WAS DISCOVERED LATER THAT THE ACTUAL INSTRUCTIONS WERE TO EXIT AT F4, HOLD SHORT OF RWY 25R, REMAIN ON THIS FREQ.) I READ BACK THE TAXI INSTRUCTIONS AS I THOUGHT I HEARD THEM. NO CORRECTION WAS MADE BY TWR. WE BOTH NOTICED AN ACFT IN THE TKOF POS BUT NOT MOVING. AS WE STARTED TO CROSS RWY 25R, I NOTED THAT THE ACFT HAD TURNED ON THEIR TKOF/LNDG LIGHTS. THE DEPARTING ACFT DID NOT HAVE TO ABORT TKOF SINCE PWR HAD NOT BEEN ADVANCED. TAPE REVEALED I NEVER SAID 'HOLD SHORT.' PERSONAL STRESS LEVEL WAS HIGH -- LOTS OF FATIGUE.
