Narrative:

After completing before start chklst, it was noticed that the fuel xfeed valve was open but not in the open position. The center tank pumps were off, which caused the EICAS fuel message to be enunciated. For the flight, the fuel was slowly drawn from the left wing fuel tank until a fuel imbal of approximately 13000 pounds existed. The EICAS message system already displayed a fuel confign message, due to the center tank pumps being in the off position, and 1200 pounds of fuel in center tank. The imbalanced condition was not noticed until a second EICAS (low fuel) message was received for having only 2200 pounds of fuel in left tank. If the first EICAS fuel confign message had been cancelled, the second fuel confign message would have been noticed when imbalance reached 2000 pound tolerance for imbalance of 2000 pounds was exceeded.

Title: B767 CREW DID NOT CLOSE THE FUEL XFEED VALVE ON COCKPIT SET UP. THE L WING TANK SLOWLY EMPTIED IN FLT UNTIL THE LOW FUEL QUANTITY EICAS WARNING WAS RECEIVED.

Narrative: AFTER COMPLETING BEFORE START CHKLST, IT WAS NOTICED THAT THE FUEL XFEED VALVE WAS OPEN BUT NOT IN THE OPEN POS. THE CTR TANK PUMPS WERE OFF, WHICH CAUSED THE EICAS FUEL MESSAGE TO BE ENUNCIATED. FOR THE FLT, THE FUEL WAS SLOWLY DRAWN FROM THE L WING FUEL TANK UNTIL A FUEL IMBAL OF APPROX 13000 LBS EXISTED. THE EICAS MESSAGE SYS ALREADY DISPLAYED A FUEL CONFIGN MESSAGE, DUE TO THE CTR TANK PUMPS BEING IN THE OFF POS, AND 1200 LBS OF FUEL IN CTR TANK. THE IMBALANCED CONDITION WAS NOT NOTICED UNTIL A SECOND EICAS (LOW FUEL) MESSAGE WAS RECEIVED FOR HAVING ONLY 2200 LBS OF FUEL IN L TANK. IF THE FIRST EICAS FUEL CONFIGN MESSAGE HAD BEEN CANCELLED, THE SECOND FUEL CONFIGN MESSAGE WOULD HAVE BEEN NOTICED WHEN IMBALANCE REACHED 2000 LB TOLERANCE FOR IMBALANCE OF 2000 LBS WAS EXCEEDED.
