Narrative:

Approach controller assigned descent from 6000 ft to 3000 ft, maintain 250 KTS, heading 290 degrees. Told to expect a visual approach to runway 12R. Approach then called our attention to traffic on climb out. PNF called traffic in sight approximately 10 NM on an eastbound course, south of our position. Approach then called a second aircraft to our attention. PNF called in sight approximately 7-8 NM at our 10-11 O'clock position. PF did not verify in sight. Approach gave us clearance to maintain visual separation. PNF acknowledge clearance. Departing aircraft was not on same frequency. PNF saw departing aircraft start a left turn to the east and assumed they would follow the same path as the first traffic. At this point we were approximately 3-4 mi from departing aircraft. Due to our close proximity to the airport and assigned speed we were descending at 2000 FPM. PF and PNF became preoccupied with descent planning and PNF lost sight of departing aircraft. Again PF never saw traffic. At approximately 5000 ft we received a 'traffic, traffic' TA followed in rapid succession by 'monitor vertical speed' and 'climb, climb,' RA's. We complied with the RA's and received 'clear of conflict.' RA's received at 4500 ft. Vertical separation was about 500 ft. We were then cleared for a visual approach to runway 12R and landed uneventfully. This was the last leg of a 12 hour duty day preceded by a 9 hour overnight and another 12 hour duty day. We were both fatigued and communication was poor. Combined with the late descent and high airspeed and the other aircraft on a different frequency, were all factors in this incident.

AT APPROX 5000 FT WE RECEIVED A 'TFC, TFC' TA FOLLOWED IN RAPID SUCCESSION BY 'MONITOR VERT SPD' AND 'CLB, CLB,' RA'S. WE COMPLIED WITH THE RA'S AND RECEIVED 'CLR OF CONFLICT.' RA'S RECEIVED AT 4500 FT. VERT SEPARATION WAS ABOUT 500 FT. WE WERE THEN CLRED FOR A VISUAL APCH TO RWY 12R AND LANDED UNEVENTFULLY. THIS WAS THE LAST LEG OF A 12 HR DUTY DAY PRECEDED BY A 9 HR OVERNIGHT AND ANOTHER 12 HR DUTY DAY. WE WERE BOTH FATIGUED AND COM WAS POOR. COMBINED WITH THE LATE DSCNT AND HIGH AIRSPD AND THE OTHER ACFT ON A DIFFERENT FREQ, WERE ALL FACTORS IN THIS INCIDENT.
