Narrative:

Failure to turn packs on after engine start. First officer failed to put packs on after starting engines. Passing 10000 ft, cabin altitude warning horn sounded. We leveled and requested return to 10000 ft. Aircraft pressurized slowly. When normal, we continued flight. Recommend packs be placed on before takeoff checklist so captain can challenge and reply and thus back up first officer.
