Narrative:

On missed approach in indianapolis after all night flight. During the go around at 1000 ft AGL captain forgot to call for climb power. This resulted in autoplt remaining in go around mode. Captain disengaged autoplt because of this confusion. When he disengaged autoplt he began to sink. At approximately 900 ft MSL GPWS activated (terrain, terrain). Because of these distractions we deviated from our assigned heading. We finally got things back under control and went to our alternate. Contributing factors: fatigue due to late night flight, ATC giving instructions for headings and altitudes at 1000 ft AGL (where climb thrust is normally called for). This distraction probably caused the captain to miss making the climb thrust call.

Title: FATIGUED B757 VARIES FROM ASSIGNED ALT AND HEADING DURING GAR AT IND.

Narrative: ON MISSED APCH IN INDIANAPOLIS AFTER ALL NIGHT FLT. DURING THE GAR AT 1000 FT AGL CAPT FORGOT TO CALL FOR CLB PWR. THIS RESULTED IN AUTOPLT REMAINING IN GAR MODE. CAPT DISENGAGED AUTOPLT BECAUSE OF THIS CONFUSION. WHEN HE DISENGAGED AUTOPLT HE BEGAN TO SINK. AT APPROX 900 FT MSL GPWS ACTIVATED (TERRAIN, TERRAIN). BECAUSE OF THESE DISTRACTIONS WE DEVIATED FROM OUR ASSIGNED HEADING. WE FINALLY GOT THINGS BACK UNDER CTL AND WENT TO OUR ALTERNATE. CONTRIBUTING FACTORS: FATIGUE DUE TO LATE NIGHT FLT, ATC GIVING INSTRUCTIONS FOR HEADINGS AND ALTS AT 1000 FT AGL (WHERE CLB THRUST IS NORMALLY CALLED FOR). THIS DISTR PROBABLY CAUSED THE CAPT TO MISS MAKING THE CLB THRUST CALL.
