Narrative:

First officer called for clearance and missed the first part. He read back what he got and asked ground to repeat the first part. Ground repeated 'radar vectors to fillmore, rest correct.' my first officer read the whole clearance back 'runway heading, to 4000 ft, radar vectors to fillmore....' after takeoff, departure control asked why we didn't turn to a 120 degree heading at 500 ft, and we stated that we were cleared runway heading till radar vectors fillmore.
