Narrative:

It has been brought to my attention on sep/xa/98 that an inspector from xyz reported that the 4H engine outboard hydraulic bay door forward hinge was not repaired per my signoff on a write-up generated at B check in ZZZ on aug/xc/98. My signoff stated that I replaced hardware and bushing. On inspecting the hinge it was found that the bracket holes were worn out causing the bushings to not fit. It was decided to leave the bushings out and generate a technical foreman item to replace the forward hinge brackets. Reference deferred item xyxy. Door was tight and secured.

Title: A B767 WAS DISPATCHED WITH AN INCOMPLETE REPAIR ACCOMPLISHED TO THE L OUTBOARD PYLON HYD BAY DOOR FORWARD HINGE.

Narrative: IT HAS BEEN BROUGHT TO MY ATTN ON SEP/XA/98 THAT AN INSPECTOR FROM XYZ RPTED THAT THE 4H ENG OUTBOARD HYD BAY DOOR FORWARD HINGE WAS NOT REPAIRED PER MY SIGNOFF ON A WRITE-UP GENERATED AT B CHK IN ZZZ ON AUG/XC/98. MY SIGNOFF STATED THAT I REPLACED HARDWARE AND BUSHING. ON INSPECTING THE HINGE IT WAS FOUND THAT THE BRACKET HOLES WERE WORN OUT CAUSING THE BUSHINGS TO NOT FIT. IT WAS DECIDED TO LEAVE THE BUSHINGS OUT AND GENERATE A TECHNICAL FOREMAN ITEM TO REPLACE THE FORWARD HINGE BRACKETS. REF DEFERRED ITEM XYXY. DOOR WAS TIGHT AND SECURED.
