Narrative:

I received the OK333 hazardous cargo slip from the ground personnel. He advised me that the shipment may or may not make the flight. When it did make the flight, I reminded him to be sure to have it entered in the load closeout. He said that he would. He seemed very professional. After the batteries were loaded, we departed the gate and did not receive the restr articles summary on the load closeout. I called bhm operations, and the agent then read me the entire closeout form -- which I already had -- again without the restr articles summary. I again told her what I needed. We departed bhm. En route to dfw, I called bhm with no response. I sent messages to dispatch, and he was unable to help. I wonder how long I would have sat on the ground at bhm, if I had refused to take off without this information. Were any FARS violated? Am I required to have the restr summary on the closeout prior to takeoff? Request answers to these questions. Recommend retraining for the bhm agent. Outsourcing agent jobs might be cheaper, but it shouldn't compromise safety.

Title: MD80S ACFT WITH HAZARDOUS CARGO LOADED DIDN'T RECEIVE A RESTR ARTICLES SUMMARY ON THE LOAD CLOSEOUT.

Narrative: I RECEIVED THE OK333 HAZARDOUS CARGO SLIP FROM THE GND PERSONNEL. HE ADVISED ME THAT THE SHIPMENT MAY OR MAY NOT MAKE THE FLT. I CALLED BHM OPS, AND THE AGENT THEN READ ME THE ENTIRE CLOSEOUT FORM -- WHICH I ALREADY HAD -- AGAIN WITHOUT THE RESTR ARTICLES SUMMARY. I AGAIN TOLD HER WHAT I NEEDED. WE DEPARTED BHM. ENRTE TO DFW, I CALLED BHM WITH NO RESPONSE. I SENT MESSAGES TO DISPATCH, AND HE WAS UNABLE TO HELP. I WONDER HOW LONG I WOULD HAVE SAT ON THE GND AT BHM, IF I HAD REFUSED TO TAKE OFF WITHOUT THIS INFO. WERE ANY FARS VIOLATED? AM I REQUIRED TO HAVE THE RESTR SUMMARY ON THE CLOSEOUT PRIOR TO TKOF?
