Narrative:

We had a last min passenger with a large bag. I took the bag to the rear of the aircraft. The ramper opened the cargo door and I placed the bag in the bin. She tagged it and closed the door and I went back to the cabin to prepare for taxi. After takeoff the cargo door opened. I declared an emergency and we landed on runway 21. There was no injury or loss of bags or cargo. The aircraft was damaged. Also there was never any cockpit indication of an open door. The cargo door annunciator never lit. Supplemental information from acn 365187: we found later there was a failure of the cargo door annunciator and that is why we had no indication.
