Narrative:

We filed the kodix 2 departure out of sdq to mia which calls for a left turn after departure to intercept W4 (351 degree radial off of cdo). Clearance we received was 'as filed, climb to FL310, cleared for takeoff.' no departure frequency given and no further instructions. After takeoff we noticed 2 aircraft to our left at approximately same altitude and 5 mi away on TCASII. We visually looked and never did see the other aircraft. We monitored our turn in relation to the TCASII display and were about to ask the tower for a frequency change when tower excitedly called and switched us to sdq center on 119.3. Changed frequency and sdq center controller very excitedly asked why we had turned left after takeoff without clearance. We explained our clearance and he went quiet. I believe contributing factor in confusion is the language difference between us and them. Sdq tower never gave us the clearance that sdq center wanted us to receive. Never got closer than 5 mi to other aircraft. Supplemental information from acn 365010: we flew the departure as published. After switching to sdq center, they notified us that we were supposed to maintain runway heading after departure. At that time we were level at 4000 ft MSL. We were instructed to immediately turn right approximately 30 degrees to heading 020 degrees. There was also some discussion about opposite traffic at the same altitude.

SUPPLEMENTAL INFO FROM ACN 365010: WE FLEW THE DEP AS PUBLISHED. AFTER SWITCHING TO SDQ CTR, THEY NOTIFIED US THAT WE WERE SUPPOSED TO MAINTAIN RWY HDG AFTER DEP. AT THAT TIME WE WERE LEVEL AT 4000 FT MSL. WE WERE INSTRUCTED TO IMMEDIATELY TURN R APPROX 30 DEGS TO HDG 020 DEGS. THERE WAS ALSO SOME DISCUSSION ABOUT OPPOSITE TFC AT THE SAME ALT.
