Narrative:

Taxi for takeoff from phl for runway 27L. Called for flaps 5 taxi clearance. First officer was receiving clearance for taxi. Frequency was busy as we taxied out for takeoff. Checklist was called complete as we were cleared for takeoff. As power was added, warning horn sounded and captain took aircraft and performed low speed rejection at about 50 KTS. We cleared runway and performed rejection checklist. Flaps were not in the 5 degree position which caused warning horn. I suspect that first officer was busy on radio and skipped the flaps call on checklist and I, as captain, did not verify 'flaps 5' as this response was never called out on checklist. Both pilots are new to B737-300 and the internal and external checklist and radio responses at times seem saturated coming from a 3 man crew on the B727. Somehow the flaps were missed on 2 checklists. I, as captain, assume that when checklist is said to be complete -- it is complete. This was not true. The takeoff warning horn prevented any further progress. I will now make sure flaps are set as part of my 'personal' checklist. The problem here is a checklist was said to be complete by first officer and in fact it was not! The old question how to make sure checklists are in fact complete remains a human factor problem. My personal solution is to slow down the checklist evolution in busy ground operations. The takeoff warning horn has been installed because of this problem and repeated occurrences.

THE OLD QUESTION HOW TO MAKE SURE CHKLISTS ARE IN FACT COMPLETE REMAINS A HUMAN FACTOR PROB. MY PERSONAL SOLUTION IS TO SLOW DOWN THE CHKLIST EVOLUTION IN BUSY GND OPS. THE TKOF WARNING HORN HAS BEEN INSTALLED BECAUSE OF THIS PROB AND REPEATED OCCURRENCES.
