Narrative:

Deicing done on west side of airport prior to takeoff. Ground man requested slats and flaps be retracted. After deicing was completed, neither I nor the copilot remembered that the flaps and slats had been retracted and did not run the complete before takeoff checklist. We simply completed the items not accomplished before deicing. On advancing the throttles for takeoff, the slat warning was received. I aborted the takeoff, exited the runway, and reaccomplished the before takeoff checklist.

Title: AN MD80 FLC ATTEMPTED TKOF WITH INCORRECTLY CONFIGURED FLAPS AND SLATS. AFTER THE PWR LEVERS WERE ADVANCED, THE FLC RECEIVED A SLAT WARNING, AND ABORTED TKOF.

Narrative: DEICING DONE ON W SIDE OF ARPT PRIOR TO TKOF. GND MAN REQUESTED SLATS AND FLAPS BE RETRACTED. AFTER DEICING WAS COMPLETED, NEITHER I NOR THE COPLT REMEMBERED THAT THE FLAPS AND SLATS HAD BEEN RETRACTED AND DID NOT RUN THE COMPLETE BEFORE TKOF CHKLIST.
