Narrative:

On arrival at dfw, the #1 flight attendant handed a 'hazardous goods notification form' to me, and apologized for not getting it to me sooner. She explained that the agent had given it to her just before closing the door, and in the rush, she had neglected to give it to me. Upon inspection, I noticed that the required signature in the 'loaded by' space was missing. The cargo was removed from the aircraft at dfw.

Title: FAR -- FLT OPS MANUAL -- UPON ARR THE LEAD FLT ATTENDANT GAVE THE CAPT AN UNSIGNED HAZMAT FORM FROM THE DEP ARPT.

Narrative: ON ARR AT DFW, THE #1 FLT ATTENDANT HANDED A 'HAZARDOUS GOODS NOTIFICATION FORM' TO ME, AND APOLOGIZED FOR NOT GETTING IT TO ME SOONER. SHE EXPLAINED THAT THE AGENT HAD GIVEN IT TO HER JUST BEFORE CLOSING THE DOOR, AND IN THE RUSH, SHE HAD NEGLECTED TO GIVE IT TO ME. UPON INSPECTION, I NOTICED THAT THE REQUIRED SIGNATURE IN THE 'LOADED BY' SPACE WAS MISSING. THE CARGO WAS REMOVED FROM THE ACFT AT DFW.
