Narrative:
I called dubuque tower, reported 9 mi west passing 3 mi north of the field. Dbq tower said 'report 3 mi west.' few mins later, dbq asked where I am, I reported 5 mi west passing 3 mi north of field. I failed to report 3 mi west because dbq had asked my position a min earlier. As I was just about north of the field, dbq ATC cleared an airlines ATR to depart on runway 36. I could not see the ATR on the ground or taking off, but within seconds I saw it to my right. I immediately executed a left turn to avoid a collision. Also, on initial contact, I reported altitude at 2800 ft MSL, again at second contact.
Original NASA ASRS Text
Title: NMAC IN CLASS D AIRSPACE.
Narrative: I CALLED DUBUQUE TWR, RPTED 9 MI W PASSING 3 MI N OF THE FIELD. DBQ TWR SAID 'RPT 3 MI W.' FEW MINS LATER, DBQ ASKED WHERE I AM, I RPTED 5 MI W PASSING 3 MI N OF FIELD. I FAILED TO RPT 3 MI W BECAUSE DBQ HAD ASKED MY POS A MIN EARLIER. AS I WAS JUST ABOUT N OF THE FIELD, DBQ ATC CLRED AN AIRLINES ATR TO DEPART ON RWY 36. I COULD NOT SEE THE ATR ON THE GND OR TAKING OFF, BUT WITHIN SECONDS I SAW IT TO MY R. I IMMEDIATELY EXECUTED A L TURN TO AVOID A COLLISION. ALSO, ON INITIAL CONTACT, I RPTED ALT AT 2800 FT MSL, AGAIN AT SECOND CONTACT.
