Narrative:

I was PIC of flight operating out of jfk on jul/xx/95. The flight was bound for bwi with a scheduled departure time of XA10 local. The flight departed the gate on time and we taxied to runway 4L for departure. We were cleared for departure at approximately XA35 and prior to attaining takeoff decision speed, the emergency exit hatch that is located above the first officer position in the cockpit came loose and blew off of the aircraft. I called for an aborted takeoff and we exited the runway at the intersection of runway 31L. We received taxi instructions and returned the aircraft to the gate where the passenger were deplaned. The type of airplane was a shorts 360. The aircraft had received a phase check the night before. The checklist completed before starting the engines included a check of the exit hatch and the first officer stating that it was secure.

Title: FLC OF AN LTT ABORTED TKOF WHEN THE COCKPIT EMER EXIT DOOR FELL OFF THE ACFT.

Narrative: I WAS PIC OF FLT OPERATING OUT OF JFK ON JUL/XX/95. THE FLT WAS BOUND FOR BWI WITH A SCHEDULED DEP TIME OF XA10 LCL. THE FLT DEPARTED THE GATE ON TIME AND WE TAXIED TO RWY 4L FOR DEP. WE WERE CLRED FOR DEP AT APPROX XA35 AND PRIOR TO ATTAINING TKOF DECISION SPD, THE EMER EXIT HATCH THAT IS LOCATED ABOVE THE FO POS IN THE COCKPIT CAME LOOSE AND BLEW OFF OF THE ACFT. I CALLED FOR AN ABORTED TKOF AND WE EXITED THE RWY AT THE INTXN OF RWY 31L. WE RECEIVED TAXI INSTRUCTIONS AND RETURNED THE ACFT TO THE GATE WHERE THE PAX WERE DEPLANED. THE TYPE OF AIRPLANE WAS A SHORTS 360. THE ACFT HAD RECEIVED A PHASE CHK THE NIGHT BEFORE. THE CHKLIST COMPLETED BEFORE STARTING THE ENGS INCLUDED A CHK OF THE EXIT HATCH AND THE FO STATING THAT IT WAS SECURE.
