Narrative:

While assigned as tower controller in charge on the shift at approximately XA00 am I approved a vehicle onto the field to perform a lighting inspection. Due to personal and family obligations I had not rested during the day prior to work, this deprivation caused me to fall asleep while on position -- even while doing a puzzle to stay awake. At approximately XA15 am approach switched the B727 to the tower on approximately a 4 mi final. I didn't hear the pilot until he was on a 2 mi final at which time I cleared him to land (forgetting the vehicle). He didn't hear me and called me back on short final and as I was clearing him again he still didn't hear me and began a go around and frequency change back to approach. I did scan the runway as I was clearing him and although the runway was clean I had still forgotten the vehicle. When he made his 2ND approach and landed -- again clean runway scan, taxiing to park, the vehicle crossed the intersecting runway. At that point I remembered. Recently alb instituted a new procedure wherein the approach controller (on mids) is to flash and or manually handoff all arrs to the tower. This procedure was not accomplished. 1) priorities -- proper rest to prevent sleep deprivation. 2) allowing vehicles access to the field without reporting points to remind me. 3) use of procedure to handoff as a safety valve for awareness.

Title: B727 MADE A GAR DUE TO NOT RECEIVING CLRNC TO LAND.

Narrative: WHILE ASSIGNED AS TWR CIC ON THE SHIFT AT APPROX XA00 AM I APPROVED A VEHICLE ONTO THE FIELD TO PERFORM A LIGHTING INSPECTION. I DID SCAN THE RWY AS I WAS CLRING HIM AND ALTHOUGH THE RWY WAS CLEAN I HAD STILL FORGOTTEN THE VEHICLE. WHEN HE MADE HIS 2ND APCH AND LANDED -- AGAIN CLEAN RWY SCAN, TAXIING TO PARK, THE VEHICLE CROSSED THE INTERSECTING RWY. AT THAT POINT I REMEMBERED. RECENTLY ALB INSTITUTED A NEW PROC WHEREIN THE APCH CTLR (ON MIDS) IS TO FLASH AND OR MANUALLY HDOF ALL ARRS TO THE TWR. THIS PROC WAS NOT ACCOMPLISHED. 1) PRIORITIES -- PROPER REST TO PREVENT SLEEP DEPRIVATION. 2) ALLOWING VEHICLES ACCESS TO THE FIELD WITHOUT RPTING POINTS TO REMIND ME. 3) USE OF PROC TO HDOF AS A SAFETY VALVE FOR AWARENESS.
