Narrative:

Our company decided to make the aircraft manifest easier to calculate. The new manifest printed combine the empty weight and crew/kit. Our company bases its crews at out stations. The station where I am based requested a list of current aircraft weights from our maintenance department. They complied by sending us a list of weights which included the empty weights and the crew/kit weight already combined. Our company received several new aircraft in which the current weight list needed to be updated. A new list was sent and some of the weights had the crew/kit added and some did not. Then a third updated list was sent which all weights were empty weights. The list worked fine but on dec/xx/94 we forgot to add the crew/kit. This allowed our manifest calculations to be 380 pounds under actual weight. Fortunately, after recalculating the manifest we were still considerably under our gross weight and the center of gravity was not affected. I notified my company of the mistake and my director of operations reinforced the need to check the weight and balance data in the aircraft. Corrective action has been taken. All weight lists at the out stations were destroyed. The maintenance department now updates a weight list on our company computer system. The computer list displays both the empty weight and the empty weight with the crew and kit added. And those weights are to be double-checked with the weight and balance data in the aircraft.

Title: TEMPORARY 'GLITCH' IN COMMUTER'S MAINT DEPT WHEN REDESIGNING THE ACFT MANIFEST FORM.

Narrative: OUR COMPANY DECIDED TO MAKE THE ACFT MANIFEST EASIER TO CALCULATE. THE NEW MANIFEST PRINTED COMBINE THE EMPTY WT AND CREW/KIT. OUR COMPANY BASES ITS CREWS AT OUT STATIONS. THE STATION WHERE I AM BASED REQUESTED A LIST OF CURRENT ACFT WTS FROM OUR MAINT DEPT. THEY COMPLIED BY SENDING US A LIST OF WTS WHICH INCLUDED THE EMPTY WTS AND THE CREW/KIT WT ALREADY COMBINED. OUR COMPANY RECEIVED SEVERAL NEW ACFT IN WHICH THE CURRENT WT LIST NEEDED TO BE UPDATED. A NEW LIST WAS SENT AND SOME OF THE WTS HAD THE CREW/KIT ADDED AND SOME DID NOT. THEN A THIRD UPDATED LIST WAS SENT WHICH ALL WTS WERE EMPTY WTS. THE LIST WORKED FINE BUT ON DEC/XX/94 WE FORGOT TO ADD THE CREW/KIT. THIS ALLOWED OUR MANIFEST CALCULATIONS TO BE 380 LBS UNDER ACTUAL WT. FORTUNATELY, AFTER RECALCULATING THE MANIFEST WE WERE STILL CONSIDERABLY UNDER OUR GROSS WT AND THE CTR OF GRAVITY WAS NOT AFFECTED. I NOTIFIED MY COMPANY OF THE MISTAKE AND MY DIRECTOR OF OPS REINFORCED THE NEED TO CHK THE WT AND BAL DATA IN THE ACFT. CORRECTIVE ACTION HAS BEEN TAKEN. ALL WT LISTS AT THE OUT STATIONS WERE DESTROYED. THE MAINT DEPT NOW UPDATES A WT LIST ON OUR COMPANY COMPUTER SYS. THE COMPUTER LIST DISPLAYS BOTH THE EMPTY WT AND THE EMPTY WT WITH THE CREW AND KIT ADDED. AND THOSE WTS ARE TO BE DOUBLE-CHKED WITH THE WT AND BAL DATA IN THE ACFT.
