In summary: aircraft had landed on runway 4. During pushback captain asked first officer to request the opposite runway for departure. Captain could not hear first officer conversation with tower due to radio/intercom design. First officer requested runway 10. Tower cleared them to the requested runway 10 and told captain they were cleared as requested. Captain taxied to runway 22. First officer did not notice. Ground controller did not notice. Cleared for takeoff. Takeoff on runway 22. Tower finally noticed but elected not to command abort. He advised of traffic on right base for runway 4 and separation was maintained.
