Narrative:

Air carrier X was climbed to 15000 ft, traffic at 16000 ft. Air carrier Y was climbed to 14000 ft and turned to put on track. Air carrier X was observed at 15100 ft. Air carrier Y was then climbed to 15000 ft. The cause of the problem was simple -- I thought air carrier X was climbing to FL230, when I saw him at 15100 ft.

Title: ACR Y CLB TO OCCUPIED ALT HAD LTSS FROM ACR X. SYS ERROR.

Narrative: ACR X WAS CLBED TO 15000 FT, TFC AT 16000 FT. ACR Y WAS CLBED TO 14000 FT AND TURNED TO PUT ON TRACK. ACR X WAS OBSERVED AT 15100 FT. ACR Y WAS THEN CLBED TO 15000 FT. THE CAUSE OF THE PROB WAS SIMPLE -- I THOUGHT ACR X WAS CLBING TO FL230, WHEN I SAW HIM AT 15100 FT.
