Narrative:

I had organized the air rides for father's day with donations going to grand rapids search and rescue association. When one of the planes taxied in and shut down, a bystander walked over to the pilot and idented himself as an aviation safety inspector. He then asked for the pilot's license number with the reason being that the one front passenger did not have his should harness on and there were 2 small children in one seat belt in the back seat. He then started walking when another pilot 'I had helping with loading' asked if he was from the FAA. He stopped and said no and then left. As he left he said he was going to report the pilot. He is, in fact, an aviation safety counselor for msp flight standards district office. I talked to him the next day and asked what he was going to do with the pilot's license number and he stated he had already reported it to the FAA and asked me at that time if I had sent notice of air rides to be given to the FAA. I had not done this so I assume he will also report this, which is why I filled out this form.

Title: PVT PLT OF SMA SINGLE ENG LAND CHARITABLE AIRLIFT FAILED TO HAVE PAX PROPERLY FASTENED WITH SEAT BELTS AND SHOULDER HARNESS. IN ADDITION, THE DIRECTOR OF THE AIRLIFT FAILED TO NOTIFY THE FAA WITHIN 7 DAYS OF THE AIRLIFT.

Narrative: I HAD ORGANIZED THE AIR RIDES FOR FATHER'S DAY WITH DONATIONS GOING TO GRAND RAPIDS SEARCH AND RESCUE ASSOCIATION. WHEN ONE OF THE PLANES TAXIED IN AND SHUT DOWN, A BYSTANDER WALKED OVER TO THE PLT AND IDENTED HIMSELF AS AN AVIATION SAFETY INSPECTOR. HE THEN ASKED FOR THE PLT'S LICENSE NUMBER WITH THE REASON BEING THAT THE ONE FRONT PAX DID NOT HAVE HIS SHOULD HARNESS ON AND THERE WERE 2 SMALL CHILDREN IN ONE SEAT BELT IN THE BACK SEAT. HE THEN STARTED WALKING WHEN ANOTHER PLT 'I HAD HELPING WITH LOADING' ASKED IF HE WAS FROM THE FAA. HE STOPPED AND SAID NO AND THEN LEFT. AS HE LEFT HE SAID HE WAS GOING TO RPT THE PLT. HE IS, IN FACT, AN AVIATION SAFETY COUNSELOR FOR MSP FLT STANDARDS DISTRICT OFFICE. I TALKED TO HIM THE NEXT DAY AND ASKED WHAT HE WAS GOING TO DO WITH THE PLT'S LICENSE NUMBER AND HE STATED HE HAD ALREADY RPTED IT TO THE FAA AND ASKED ME AT THAT TIME IF I HAD SENT NOTICE OF AIR RIDES TO BE GIVEN TO THE FAA. I HAD NOT DONE THIS SO I ASSUME HE WILL ALSO RPT THIS, WHICH IS WHY I FILLED OUT THIS FORM.
