Narrative:

I unknowingly operated an aircraft in IMC conditions while on an IFR flight plan, that due to a recent maintenance installment, the aircraft was not suitable for IFR/IMC conditions. See attached paperwork. This occurred between april and june 1994. Supplemental information from acn 274499: company installed electric led readout vsi on captain and first officer's side, in anticipation of TCASII being installed. These instruments would continually fail in-flight causing a blank screen. An FAA inspector was riding along (another flight) and noticed the failures, and grounded fleet to day VFR flight plans only. Corrective action by company: file VFR flight plans until old vsi can be re-installed or problem corrected.

THESE INSTS WOULD CONTINUALLY FAIL INFLT CAUSING A BLANK SCREEN. AN FAA INSPECTOR WAS RIDING ALONG (ANOTHER FLT) AND NOTICED THE FAILURES, AND GNDED FLEET TO DAY VFR FLT PLANS ONLY. CORRECTIVE ACTION BY COMPANY: FILE VFR FLT PLANS UNTIL OLD VSI CAN BE RE-INSTALLED OR PROB CORRECTED.
