Narrative:

Night operation -- runway 9R under construction -- threshold relocated 4478 ft east. Air carrier X landed runway 9R. Air carrier Y was cleared to land runway 12 to hold short of runway 9R. Air carrier Z was cleared to land runway 9R and advised traffic landing runway 12 will hold short of your runway. During the arrival push, air carrier X landed runway 9R. Air carrier X was instructed to cross runway 12, turn left at the end and contact ground. Air carrier X turned onto runway 12 (opposite direction) and stopped momentarily. Realizing his mistake, the pilot turned back onto runway 9R. The pilot was instructed to turn at the end and exit the runway. Local controller, observing traffic on runway 12. Air carrier Y was issued a go around and given missed approach instructions. Air carrier Z observed traffic on the runway and initiated a go around. Air carrier Y and Z were on a converging course. Air carrier Z executed an evasive maneuver to avoid air carrier Y. Callback conversation with reporter revealed the following information: reporter stated aircraft involved air carrier X, F-28, air carrier Y and Z were B-727's. Reporter was working ground control and notified the local controller about air carrier X runway incursion. Local controller issued go around to air carrier Y. Air carrier Z flight crew initiated go around. Local controller did not issue instructions to air carrier Z. Flight crew ATC review captain of air carrier Z said he saw what was happening and had several plans to avoid air carrier Y.

ACR Z WAS CLRED TO LAND RWY 9R AND ADVISED TFC LNDG RWY 12 WILL HOLD SHORT OF YOUR RWY. DURING THE ARR PUSH, ACR X LANDED RWY 9R. ACR X WAS INSTRUCTED TO CROSS RWY 12, TURN L AT THE END AND CONTACT GND. ACR X TURNED ONTO RWY 12 (OPPOSITE DIRECTION) AND STOPPED MOMENTARILY. REALIZING HIS MISTAKE, THE PLT TURNED BACK ONTO RWY 9R. THE PLT WAS INSTRUCTED TO TURN AT THE END AND EXIT THE RWY. LCL CTLR, OBSERVING TFC ON RWY 12. ACR Y WAS ISSUED A GAR AND GIVEN MISSED APCH INSTRUCTIONS. ACR Z OBSERVED TFC ON THE RWY AND INITIATED A GAR. ACR Y AND Z WERE ON A CONVERGING COURSE. ACR Z EXECUTED AN EVASIVE MANEUVER TO AVOID ACR Y. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED ACFT INVOLVED ACR X, F-28, ACR Y AND Z WERE B-727'S. RPTR WAS WORKING GND CTL AND NOTIFIED THE LCL CTLR ABOUT ACR X RWY INCURSION. LCL CTLR ISSUED GAR TO ACR Y. ACR Z FLC INITIATED GAR. LCL CTLR DID NOT ISSUE INSTRUCTIONS TO ACR Z. FLC ATC REVIEW CAPT OF ACR Z SAID HE SAW WHAT WAS HAPPENING AND HAD SEVERAL PLANS TO AVOID ACR Y.
