Narrative:

Air carrier X was eastbound from hnl to sea via A331 at FL330. Air carrier Y was wbound from lax to taipei via alcoa 38/130 37/140 37/150 38/160 at FL310. At XX40Z OC7 was combined at OC1, with the OC7 controller moving to OC1. At XX41Z air carrier Y requested FL330. The OC1/7 controller conducted a traffic search by scanning the flight progress strips and the odaps pvd. At XX43Z, the OC1/7 controller issued air carrier Y a climb clearance to FL330. At XY17Z separation was lost between air carrier X and air carrier Y. During a later scan of the sector, the OC1/7 controller discovered that the 140W flight progress strip of air carrier Y had been mis-sequenced under the wrong bay header. When the flight progress strip was placed under the correct bay header, the OC1/7 controller realized that air carrier Y was in conflict with air carrier X. At XY31Z the controller called hnl arinc, but found that air carrier Y was still with sfo arinc. At XY32:45Z the OC1/7 controller called sfo arinc and issued a descent clearance for air carrier Y to descend and maintain FL310 for traffic. Sfo arinc issued the clearance but advised that air carrier Y reported passing the traffic and requested to remain at FL330. At XY36Z the OC1/7 controller requested the position of air carrier X and air carrier Y at XY46Z air carrier X gave their position and advised they had received a TCASII alert and had seen a widebody transport passing from east to west about 10 to 12 mins ago. Lateral separation was re-established at XY40Z.

Title: ACR Y CLB TO OCCUPIED ALT ON OCEANIC RTE, HAD LTSS FROM ACR X SYS ERROR.

Narrative: ACR X WAS EBOUND FROM HNL TO SEA VIA A331 AT FL330. ACR Y WAS WBOUND FROM LAX TO TAIPEI VIA ALCOA 38/130 37/140 37/150 38/160 AT FL310. AT XX40Z OC7 WAS COMBINED AT OC1, WITH THE OC7 CTLR MOVING TO OC1. AT XX41Z ACR Y REQUESTED FL330. THE OC1/7 CTLR CONDUCTED A TFC SEARCH BY SCANNING THE FLT PROGRESS STRIPS AND THE ODAPS PVD. AT XX43Z, THE OC1/7 CTLR ISSUED ACR Y A CLB CLRNC TO FL330. AT XY17Z SEPARATION WAS LOST BTWN ACR X AND ACR Y. DURING A LATER SCAN OF THE SECTOR, THE OC1/7 CTLR DISCOVERED THAT THE 140W FLT PROGRESS STRIP OF ACR Y HAD BEEN MIS-SEQUENCED UNDER THE WRONG BAY HEADER. WHEN THE FLT PROGRESS STRIP WAS PLACED UNDER THE CORRECT BAY HEADER, THE OC1/7 CTLR REALIZED THAT ACR Y WAS IN CONFLICT WITH ACR X. AT XY31Z THE CTLR CALLED HNL ARINC, BUT FOUND THAT ACR Y WAS STILL WITH SFO ARINC. AT XY32:45Z THE OC1/7 CTLR CALLED SFO ARINC AND ISSUED A DSCNT CLRNC FOR ACR Y TO DSND AND MAINTAIN FL310 FOR TFC. SFO ARINC ISSUED THE CLRNC BUT ADVISED THAT ACR Y RPTED PASSING THE TFC AND REQUESTED TO REMAIN AT FL330. AT XY36Z THE OC1/7 CTLR REQUESTED THE POS OF ACR X AND ACR Y AT XY46Z ACR X GAVE THEIR POS AND ADVISED THEY HAD RECEIVED A TCASII ALERT AND HAD SEEN A WDB PASSING FROM E TO W ABOUT 10 TO 12 MINS AGO. LATERAL SEPARATION WAS RE-ESTABLISHED AT XY40Z.
