Narrative:

Aircraft was to takeoff using 5 degrees flaps. Flaps were extended to 2 degrees in error. Upon being cleared for takeoff, final items on checklist were being completed. Simultaneously, power was being advanced for takeoff. Warning horn sounded indicating flaps were not at least at 5 degree position. Pulled off runway, got back in lineup for takeoff, and recompleted takeoff check. We should have caught this error during initial takeoff check. More vigilance should have been used when completing checklists. Repetitive nature of checklists lends itself to being less vigilant. One must recognize this fact.

Narrative: ACFT WAS TO TKOF USING 5 DEGS FLAPS. FLAPS WERE EXTENDED TO 2 DEGS IN ERROR. UPON BEING CLRED FOR TKOF, FINAL ITEMS ON CHKLIST WERE BEING COMPLETED. WE SHOULD HAVE CAUGHT THIS ERROR DURING INITIAL TKOF CHK. MORE VIGILANCE SHOULD HAVE BEEN USED WHEN COMPLETING CHKLISTS. REPETITIVE NATURE OF CHKLISTS LENDS ITSELF TO BEING LESS VIGILANT. ONE MUST RECOGNIZE THIS FACT.
