Narrative:

Given IFR clearance to maintain 4000 ft until advised by center. Did not contact center until leaving 5200 ft. Reported climbing to 6000 ft (the usual altitude we get). Center asked what I was cleared to, then I realized problem. I told center I was VFR and could cancel IFR and pick up IFR clearance again over chain intersection. (The way we depart 95 percent of the time.) contributing factors: fatigue -- 2 hours sleep the night before, 5.5 hours block time and 10 takeoffs before this. Distracted -- was 45 mins late due to missing approach into airport, trying to call company while climbing out -- should have waited. Did not write down clearance and review it with PF. Did not call center at 2000 ft which is normal.
