Narrative:

On apr/xx/93, I was involved on outboard flap toggle installation at phl international airport based air carrier. I'm reporting the installation was correct with appropriate paper work to the best of my knowledge. I'm writing this report due to fact aircraft landed at ind, with the same write-up, after I installed 1 bolt, roller and castle nut with cotter pin installed. There may have been a bushing missing on the toggle area which I didn't remove which caused a duplicate write-up. To prevent a recurrence, there should be an inspector available to check the whole operation after the fact. There were 2 chapters or areas or paper work on the job.

Title: MECH RPTS IMPROPERLY INSTALLED OUTBOARD FLAP TOGGLE, MISSING A BUSHING.

Narrative: ON APR/XX/93, I WAS INVOLVED ON OUTBOARD FLAP TOGGLE INSTALLATION AT PHL INTL ARPT BASED ACR. I'M RPTING THE INSTALLATION WAS CORRECT WITH APPROPRIATE PAPER WORK TO THE BEST OF MY KNOWLEDGE. I'M WRITING THIS RPT DUE TO FACT ACFT LANDED AT IND, WITH THE SAME WRITE-UP, AFTER I INSTALLED 1 BOLT, ROLLER AND CASTLE NUT WITH COTTER PIN INSTALLED. THERE MAY HAVE BEEN A BUSHING MISSING ON THE TOGGLE AREA WHICH I DIDN'T REMOVE WHICH CAUSED A DUPLICATE WRITE-UP. TO PREVENT A RECURRENCE, THERE SHOULD BE AN INSPECTOR AVAILABLE TO CHK THE WHOLE OPERATION AFTER THE FACT. THERE WERE 2 CHAPTERS OR AREAS OR PAPER WORK ON THE JOB.
