Narrative:

Report of fatigue on a 3 day trip. Trip was characterized by 1ST day -- I leg. 2ND day -- 8 legs (12 hour duty). 3RD day -- 3 legs (6 hour duty). Rest of 8:16 scheduled between 2ND and 3RD day. Transportation to/from hotel equals 30 mins each way. Chkin/chkout, wind down time, shave/shower account for another 1 hour. Stop and got dinner at restaurant on way to hotel -- 15 mins. Result is 6 hours rest (maximum) following 12 hour day with multiple approachs, followed by 6 hour day on 3RD day. Felt tired/fatigued on last leg. Behind aircraft tasks and workload. Not safe! Someone is going to get hurt.

SOMEONE IS GOING TO GET HURT.
