Narrative:

During taxi nose gear collapsed on taxiway. Was determined later that the nose gear release locking pin was not secured in place by the aircraft required crew member. Our operating limitations and flight manual for this aircraft require and consist of a min of 1 pilot and 1 crew member familiar with the mechanical system of the aircraft. Normally the aircraft is preflted by the pilot and he delegates functions to the required crew member. Normally after preflight is completed, routinely the aircraft is towed to a new position for startup with the pilot at the controls for brakes if required during the tow. The crew member then reconnects the nose gear locking pin after the tow bar is removed and acknowledges that preflight is completed to the pilot, which was so acknowledged that day, before the incident occurred. Even though the crew member had training and 2 yrs of experience as a crew member on this aircraft and holds a pilots license, the locking pin was not secured in place. To prevent this occurrence, a flag should be placed on the throttles or controls, warning, check nose gear locking pin in place and an additional flag on nose gear pin for the pilot and crew member to check each other's actions. The pilot should check the nose gear pin visually in place. An additional placard lettering should be placed on the nose gear strut assembly, and additional warnings added to the checklist.

Title: PRIVATELY OWNED BMB ACFT'S NOSE GEAR COLLAPSED DURING TAXI FOR TKOF.

Narrative: DURING TAXI NOSE GEAR COLLAPSED ON TAXIWAY. WAS DETERMINED LATER THAT THE NOSE GEAR RELEASE LOCKING PIN WAS NOT SECURED IN PLACE BY THE ACFT REQUIRED CREW MEMBER. NORMALLY THE ACFT IS PREFLTED BY THE PLT AND HE DELEGATES FUNCTIONS TO THE REQUIRED CREW MEMBER. NORMALLY AFTER PREFLT IS COMPLETED, ROUTINELY THE ACFT IS TOWED TO A NEW POS FOR STARTUP WITH THE PLT AT THE CTLS FOR BRAKES IF REQUIRED DURING THE TOW. THE CREW MEMBER THEN RECONNECTS THE NOSE GEAR LOCKING PIN AFTER THE TOW BAR IS REMOVED AND ACKNOWLEDGES THAT PREFLT IS COMPLETED TO THE PLT, WHICH WAS SO ACKNOWLEDGED THAT DAY, BEFORE THE INCIDENT OCCURRED. EVEN THOUGH THE CREW MEMBER HAD TRAINING AND 2 YRS OF EXPERIENCE AS A CREW MEMBER ON THIS ACFT AND HOLDS A PLTS LICENSE, THE LOCKING PIN WAS NOT SECURED IN PLACE. TO PREVENT THIS OCCURRENCE, A FLAG SHOULD BE PLACED ON THE THROTTLES OR CTLS, WARNING, CHK NOSE GEAR LOCKING PIN IN PLACE AND AN ADDITIONAL FLAG ON NOSE GEAR PIN FOR THE PLT AND CREW MEMBER TO CHK EACH OTHER'S ACTIONS. THE PLT SHOULD CHK THE NOSE GEAR PIN VISUALLY IN PLACE. AN ADDITIONAL PLACARD LETTERING SHOULD BE PLACED ON THE NOSE GEAR STRUT ASSEMBLY, AND ADDITIONAL WARNINGS ADDED TO THE CHKLIST.
