Narrative:

I work for a level 1 trauma center in xyz, medical center provides 24 hour EMS helicopter service for abc, and def. At AM00 I was notified that we would be flying a critical heart attack patient from ZZZ hospital.at AA30 I called the dispatch center at medical center for latitude/long coordinates for our LORAN receiver, as our hospital didn't have this information. The dispatcher gave me the coordinates and told me that this hospital was just north of their location. I asked her if she knew if this hospital was in the TCA and she replied 'no, I don't think it is.' I asked to speak with the pilot on duty. He said he had flown over it before but had never landed there. He thought it might be marked on the VFR helicopter route chart for YYY. We both pulled out the latest editions of this chart and noted that it was marked on the chart. We discussed the fact that this location was not in TCA and thus would not require clearance to land there. This flight was then delayed until we departed our hospital at BB54 due to hospital making arrangements for a free bed. At CC50, we departed ZZZ for hospital. I flew outside the southwest TCA boundary until we were just north of landmark a. I then proceeded north per the navigation data off my LORAN receiver. I landed at hospital at DD30. The LORAN coordinates were accurate and corresponded to the actual hospital location. At EE00 we departed hospital to the south and then flew around the TCA toward qrs. En route we received another EMS 1 EMS flight from hospital B, I noticed during my post-flight debriefing that hospital a is actually located northwest of the landmark B depicted on the helicopter chart. I've enclosed a copy of the chart with the correct location for the hospital shown in red. This would put this hospital inside the YYY TCA boundary by approximately 1 mi. Had I noticed the significance of this landmark B to the hospital before I landed there, I would have called YYY tower for clearance before landing there. Since area helicopter pilots navigate off the visual landmarks and locations as depicted on the VFR helicopter route chart for YYY, the potential exists for further unintentional incursions of the YYY TCA when landing at hospital a. I've also noted that although (unverified) is listed under hospital a on the chart, there is no definition of what exactly this means on the legend of the chart.

Title: AN EMS HELI PLT WENT TO A HOSPITAL USING HIS LORAN NAV AND HELI RTE CHART. HE FOUND THAT THE HOSPITAL WAS NOT WHERE IT WAS SHOWN ON THE HELI RTE CHART BUT WAS IN FACT INSIDE THE YYY TCA. IN A CALLBACK, THE PLT ASSURED ME THAT LORAN COVERAGE IS REPEATABLE IN NORTHERN REGION AND THAT THIS IS HIS FIRST OCCASION TO FIND SOMETHING MIS-CHARTED.

Narrative: I WORK FOR A LEVEL 1 TRAUMA CTR IN XYZ, MEDICAL CTR PROVIDES 24 HR EMS HELI SVC FOR ABC, AND DEF. AT AM00 I WAS NOTIFIED THAT WE WOULD BE FLYING A CRITICAL HEART ATTACK PATIENT FROM ZZZ HOSPITAL.AT AA30 I CALLED THE DISPATCH CTR AT MEDICAL CTR FOR LAT/LONG COORDINATES FOR OUR LORAN RECEIVER, AS OUR HOSPITAL DIDN'T HAVE THIS INFO. THE DISPATCHER GAVE ME THE COORDINATES AND TOLD ME THAT THIS HOSPITAL WAS JUST N OF THEIR LOCATION. I ASKED HER IF SHE KNEW IF THIS HOSPITAL WAS IN THE TCA AND SHE REPLIED 'NO, I DON'T THINK IT IS.' I ASKED TO SPEAK WITH THE PLT ON DUTY. HE SAID HE HAD FLOWN OVER IT BEFORE BUT HAD NEVER LANDED THERE. HE THOUGHT IT MIGHT BE MARKED ON THE VFR HELI RTE CHART FOR YYY. WE BOTH PULLED OUT THE LATEST EDITIONS OF THIS CHART AND NOTED THAT IT WAS MARKED ON THE CHART. WE DISCUSSED THE FACT THAT THIS LOCATION WAS NOT IN TCA AND THUS WOULD NOT REQUIRE CLRNC TO LAND THERE. THIS FLT WAS THEN DELAYED UNTIL WE DEPARTED OUR HOSPITAL AT BB54 DUE TO HOSPITAL MAKING ARRANGEMENTS FOR A FREE BED. AT CC50, WE DEPARTED ZZZ FOR HOSPITAL. I FLEW OUTSIDE THE SW TCA BOUNDARY UNTIL WE WERE JUST N OF LANDMARK A. I THEN PROCEEDED N PER THE NAV DATA OFF MY LORAN RECEIVER. I LANDED AT HOSPITAL AT DD30. THE LORAN COORDINATES WERE ACCURATE AND CORRESPONDED TO THE ACTUAL HOSPITAL LOCATION. AT EE00 WE DEPARTED HOSPITAL TO THE S AND THEN FLEW AROUND THE TCA TOWARD QRS. ENRTE WE RECEIVED ANOTHER EMS 1 EMS FLT FROM HOSPITAL B, I NOTICED DURING MY POST-FLT DEBRIEFING THAT HOSPITAL A IS ACTUALLY LOCATED NW OF THE LANDMARK B DEPICTED ON THE HELI CHART. I'VE ENCLOSED A COPY OF THE CHART WITH THE CORRECT LOCATION FOR THE HOSPITAL SHOWN IN RED. THIS WOULD PUT THIS HOSPITAL INSIDE THE YYY TCA BOUNDARY BY APPROX 1 MI. HAD I NOTICED THE SIGNIFICANCE OF THIS LANDMARK B TO THE HOSPITAL BEFORE I LANDED THERE, I WOULD HAVE CALLED YYY TWR FOR CLRNC BEFORE LNDG THERE. SINCE AREA HELI PLTS NAVIGATE OFF THE VISUAL LANDMARKS AND LOCATIONS AS DEPICTED ON THE VFR HELI RTE CHART FOR YYY, THE POTENTIAL EXISTS FOR FURTHER UNINTENTIONAL INCURSIONS OF THE YYY TCA WHEN LNDG AT HOSPITAL A. I'VE ALSO NOTED THAT ALTHOUGH (UNVERIFIED) IS LISTED UNDER HOSPITAL A ON THE CHART, THERE IS NO DEFINITION OF WHAT EXACTLY THIS MEANS ON THE LEGEND OF THE CHART.
