Narrative:

We had been cleared into position to hold on runway 6R while a widebody transport cleared the runway. As the widebody transport cleared the runway, we were issued an altitude restriction for departure and cleared for takeoff. I read back the restriction and said 'here we go' to the tower controller. At about 80 KTS, the tower controller said we were cleared for takeoff. I responded he had already cleared us for takeoff. He then replied that had only issued an altitude restriction for our departure and not a takeoff clearance. Both pilot's felt certain the takeoff clearance had been issued at the time the departure restriction was issued.

Title: UNAUTHORIZED UNCOORD TKOF.

Narrative: WE HAD BEEN CLRED INTO POS TO HOLD ON RWY 6R WHILE A WDB CLRED THE RWY. AS THE WDB CLRED THE RWY, WE WERE ISSUED AN ALT RESTRICTION FOR DEP AND CLRED FOR TKOF. I READ BACK THE RESTRICTION AND SAID 'HERE WE GO' TO THE TWR CTLR. AT ABOUT 80 KTS, THE TWR CTLR SAID WE WERE CLRED FOR TKOF. I RESPONDED HE HAD ALREADY CLRED US FOR TKOF. HE THEN REPLIED THAT HAD ONLY ISSUED AN ALT RESTRICTION FOR OUR DEP AND NOT A TKOF CLRNC. BOTH PLT'S FELT CERTAIN THE TKOF CLRNC HAD BEEN ISSUED AT THE TIME THE DEP RESTRICTION WAS ISSUED.
